We are committed to transparency, client rights, and ethical care. The information below explains how you can access your records, contact our licensing authority, and file a complaint if needed.

1. Requesting Your Health Care Records

You have the right to access and obtain a copy of your health care records in accordance with applicable laws.

To request your records:

  • Submit a written request via email, mail, or in person, or complete the request form on this page to initiate the verification process.

  • Provide your full name, date of birth, and specify the records requested. A completed Release of Information form is required.

  • Present a valid form of identification.

Processing Time:
In Texas, under the HIPAA Privacy Rule requests are typically responded to within 30 business days of the request.

Fees:
Reasonable fees may apply for copying, mailing, or electronic records, as permitted by law.

2. Licensing & Disciplinary Authority

    3. Filing a Consumer Complaint

    If you believe your privacy rights have been violated, you have the right to file a complaint under
    Texas Health and Safety Code §181.103
