You have the right to access and obtain a copy of your health care records in accordance with applicable laws.
To request your records:
Submit a written request via email, mail, or in person, or complete the request form on this page to initiate the verification process.
Provide your full name, date of birth, and specify the records requested. A completed Release of Information form is required.
Present a valid form of identification.
Processing Time:
In Texas, under the HIPAA Privacy Rule requests are typically responded to within 30 business days of the request.
Fees:
Reasonable fees may apply for copying, mailing, or electronic records, as permitted by law.
