Mental Health Targeted Case Management

Mental Health Targeted Case Management (MHTCM) is a service that helps people get the care and support they need. It is available through Texas Medicaid for children, teens, and adults who are living with ongoing mental health challenges.

If you receive MHTCM services, you will have a case manager who works with you one-on-one. Your case manager helps you:

  • Find mental health and medical care
  • Connect to counseling, therapy, or psychiatric services
  • Get help with school, work, housing, or community resources
  • Keep track of appointments and services
  • Feel supported while navigating systems that can feel overwhelming

You don’t have to figure everything out by yourself. Your case manager’s role is to listen, support you, and help connect you to services that fit your needs.

Services are based on an assessment to make sure this support is right for you. The goal is to help you feel more stable, supported, and confident in managing your care and daily life.

Skill Building and Development

Skill building and personal development support helps you learn or strengthen everyday skills that make daily life feel more manageable and independent. This service focuses on building confidence and practical abilities at your own pace.

Skill building support may include:

  • Learning daily living skills like routines, organization, or time management
  • Building communication and problem-solving skills
  • Practicing coping skills for stress and emotional regulation
  • Developing social skills and community connections
  • Working toward personal goals related to independence and stability

The goal is to support you in gaining skills that help you feel more confident, capable, and connected in your daily life and community. We work alongside you, focusing on your strengths and what matters most to you.

Crisis Prevention

Crisis prevention support is designed to help you feel safer, more supported, and better prepared during difficult or overwhelming moments. This service focuses on recognizing early warning signs and building simple, practical plans to help reduce stress before a situation becomes a crisis.

Crisis prevention support may include:

  • Talking through what feels overwhelming or stressful
  • Identifying triggers and early warning signs
  • Creating a personal plan for calming and coping
  • Connecting you to trusted supports and community resources
  • Helping you know when and how to reach out for additional help

Our goal is to support you in staying safe, feeling heard, and having tools in place so you’re not facing hard moments alone. We work alongside you to build confidence, stability, and support.

Medication Management

Medication support is here to help you feel more comfortable and confident with your medications. We don’t prescribe or change medications, but we can help you understand them and stay organized.

This support may include:

  • Talking through what your medications are for
  • Helping you keep track of when and how to take them
  • Supporting you in sharing questions or concerns with your doctor
  • Noticing changes or side effects and helping you reach out for help
  • Offering encouragement and reminders when things feel overwhelming

Our goal is to support you, reduce stress, and help you feel more in control of your care. You’re not expected to figure everything out on your own—we’re here to walk alongside you.
